  • PHP or Partial Hospitalization Program — This is an outpatient program, meaning that you go home each night but return to our office five days a week. You’ll spend six hours a day in group counseling in our office. You may also receive medicine to help you overcome withdrawal symptoms and cravings for drugs or alcohol. Private counseling is also available, as well as other therapies to help you learn what causes drug addiction and how you can manage your addiction effectively.
  • IOP or Intensive Outpatient Program — Intensive outpatient programs work well for many people, and they may work for you, too. Health insurances usually cover IOP (with prior authorization). In an intensive outpatient program, you’ll visit our office three days a week for three hours each day. You’ll attend classes to learn about causes of addiction and how to manage your problem, as well as developing a support network with others.

In group sessions, you’ll get a chance to share information and receive feedback, support and help by other clients with similar problems. All sessions are moderated by a trained drug and alcohol counselor with many years of experience.   • Individual counseling — Individual counseling, added to the PHP or IOP program, helps people continue the good work they started during their outpatient treatment at Synergy Recovery Services. These are private, one-to-one counseling sessions held in our professional office in Bakersfield. You’ll go once or twice a month to speak with your counselor and continue to work on your recovery. Counselors use a wide range of therapies to help our clients and will work with you to choose the one that’s right for you.

Instead, medications are now available to offset some of the worst withdrawal symptoms and help people manage drug cravings.

Dr. Trobisch may prescribe medications to help you through your initial treatment, including:

  • Buprenorphine (Suboxone) — This medication is often used to help patients overcome withdrawal symptoms and cravings in heroin or opioid addiction. Because it acts on the same areas of the brain, it works immediately to reduce cravings or withdrawals in addiction to heroin or prescription painkillers (like oxycodone, hydrocodone, morphine, fentanyl or others). We use it to help patients gradually withdraw from opioids. Over time, and with gradually monitored reduction in dose, you can be free from all opioid use.
  • Naltrexone (Vivitrol) — Vivitrol works different from Suboxone and helps block cravings for drugs and alcohol. By specifically blocking opioid receptors in your brain, Vivitrol makes you virtually “immune” to opioid use and thus prevents a relapse. Cravings often lead to relapse, so preventing cravings may help you resist a relapse. It’s given as an injection that lasts about a month, and there are specific conditions that must be met in order for you to receive the injection. Dr. Trobisch can help you decide if this medicine is right for you and discuss possible side effects and benefits during an appointment.

Other medications may be used to manage the various potential symptoms of withdrawal. Not everyone reacts the same way to withdrawal, therefore medication management is highly individualized. We work with you to manage all of your symptoms and help you overcome any unpleasant side effects, so you can continue moving forward with your recovery counseling.

How Does Addiction Happen?

Drug addiction isn’t a sign of weakness — it’s a complex illness that affects your body, mind and spirit. When you take drugs or drink alcohol, it sets up a chain reaction in your brain that makes you crave more. Depending on the drugs you take, specific brain chemicals called neurotransmitters are changed. Sometimes, drugs make you release more of certain chemicals, while others are suppressed.

Your brain and body’s chief goal, however, is health and survival. It adapts to the new flood of chemicals entering your system by changing how much of any particular neurotransmitter it makes, or changing the number of receptor sites that can receive and process the chemicals. Over time, theses adaptations make the original amount of drug useless. You start to need more to get high, and that’s when the cycle of addiction begins.

As you take more drugs, your body becomes physically dependent on a specific amount of the substance you’re using in order for you to keep functioning. If you don’t get the drugs or alcohol, you begin to feel physically sick. You may get shaky, nauseous, itchy or sweaty. Unfortunately, these sick feelings make you want more drugs just to feel “normal.”

Drugs and alcohol also cause psychological addiction. This means that you may crave drugs both for the physical sensations they produce as well as the mental effects they provide. You begin to need drugs to relax or socialize, or just to cope with daily problems. Soon, drugs take over your life.
